Hospice Team

Each one of our patients is cared for by a broad team of hospice professionals. For example, hospice physicians, Nurse Practitioners, and more. That is to say, all of these work together. Consequently, they provide the best possible hospice care to you and your loved ones.

Physician Services

The Hospice Physician can work with the attending physician to meet the general medical needs of patients and families. Moreover, hospice can work together with the physician of the patient’s choosing.

Nursing Services

The RN Case Manager puts together the patient’s care and services given by the hospice team. Also, the hospice nurse works on symptom control and pain management.

Hospice Home Health Aide

The Hospice Home Health Aide gives personal care under the supervision of the RN Case Manager. In addition, help with bathing and grooming.

Social Worker

The Social Worker gives psychological and emotional assessment of the needs of the patient and family. Information and community resources are also given.

Chaplain

Chaplains are available to help our patients and family members connect to their faith tradition. Certainly, Spiritual Care resources can help people return a sense of balance into their lives.

Bereavement

Many programs give Bereavement services. To sum up, it gives support, guidance, and education.
